Under data protection law you, as patient of Cathryn Clark Physiotherapy, you have specific rights. To communicate these rights to you in a clear and concise manner, we are providing you with this privacy notice.

We record and use the following categories of personal data: name, address, telephone numbers, email address, date of birth, health information including medial history, diagnosis and treatment data. Our lawful basis of processing this data is one of contract and, for the health information; it is necessary when providing physiotherapy for the purposes of health diagnosis and treatment. In addition, we will only examine or treat you with your explicit consent. From time to time, we would like to send you information about the clinic, our services and relevant advice; we will only do this with your specific consent.
Sharing Your Personal Data
We only share your personal data with your explicit consent, where, for example we need to contact a third party and give them your contact details in order to seek treatment authorisation and payment for treatment. Where third parties are used by us to store your personal data, we ensure they are compliant with the data protection law and any such data is not stored outside of the EU. We will not share your data with any other company or individual for the purpose of marketing.
Retaining Your Personal Data
Whilst you are receiving treatment from us we will continue to store and use your personal data. Once your treatment has been completed, we will be legally required to retain your personal data for eight years from the date of your last treatment.
Your Rights
As we process your personal data, you have certain rights. These are a right of access, a right of rectification, a right of erasure and a right to restrict processing.

How long we retain your data
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
